Help:Ops/templates/address mismatch

Greetings!

I now have your [description of items] ready to ship.

However, when I checked the credit card number you gave us, the card processing company said that the address they had on file did not match the address you gave us:

Could you please check your most recent credit card statement (or bank statement, if the card is a check card) and verify that the address is correct? If the address is different, please let me know what the correct address is and I will finish processing your order right away.

If it is correct as far as you can tell, please send me the name and telephone number of the issuing bank (usually on the back of the card); I should be able to verify the address with them.
